Definition
Therapeutic peptides are short-chain amino acids that regulate cellular functions and facilitate biochemical processes, and their global market and popularity among patients have grown significantly in recent years. [1] Peptide therapeutics have played a notable role in medical practice since the advent of insulin therapy in the 1920s, and over 60 peptide drugs are approved in the United States and other major markets. [2] Peptides are recognized for being highly selective and efficacious while at the same time relatively safe and well tolerated, which has driven increased interest in peptides in pharmaceutical research and development. [3] Alongside legitimate regulated peptide medicines, online biohacking and wellness spaces increasingly promote experimental or weakly evidenced peptides for fat loss, recovery, aesthetics, cognition, performance, and longevity outside clinical supervision. [4]
How it works
Peptide therapeutics engage defined receptors and intracellular cascades, providing target selectivity typically associated with parenterally delivered monoclonal antibodies. [5] [6] In gerontology, therapeutic peptides target fundamental hallmarks of aging including metabolic dysfunction, telomere attrition, tissue repair impairment, and hormonal decline. [7] The development of peptide therapeutics has historically been limited by the lack of drug-like properties of peptides, including poor in vivo stability, poor solubility, and incompatibility with oral administration. [8] Protein and peptide therapeutics generally require parenteral administration, and extensive efforts have targeted alternative delivery strategies such as permeation enhancers, gut enzyme inhibitors, and cell-penetrating peptides. [9]
Evidence & status
Approximately 140 peptide therapeutics were being evaluated in clinical trials, reflecting an established and active development pipeline. [3] A landmark advance was the approval of an oral formulation of the glucagon-like peptide 1 receptor agonist semaglutide for the treatment of type 2 diabetes. [9] In a gerontology review, FDA-approved peptide agents demonstrated robust safety profiles from large-scale trials, whereas non-approved peptides showed promising preclinical and limited clinical evidence but lacked long-term safety data and systematic validation. [7] For popular injectable peptides used in orthopaedics such as BPC-157, benefits in tendon and muscle repair are largely unvalidated in human trials, and information on indications, dosing, frequency, and duration of treatment remains unknown. [1] The central safety concern with unregulated peptide use is consumer experimentation with products of uncertain identity, purity, potency, sterility, and safety, compounded by self-injection, stacking, and weak pharmacovigilance. [4]
Applications
Recent technical advances have led to rapid growth in the development of peptides for a wide range of therapeutic indications such as diabetes, cancer, and pain. [8] In gerontology, identified peptides span metabolic restoration (tirzepatide), telomere biology (epitalon), dermal regeneration (GHK-Cu), tissue repair (BPC-157, TB-500), neuroprotection (Semax), growth hormone modulation (CJC-1295, ipamorelin), and sexual function (bremelanotide). [7] Peptide-based strategies are being investigated for central nervous system repair in neurodegenerative conditions such as Alzheimer's and Parkinson's disease, with early human trials supporting feasibility and safety. [5] Next-generation approaches include peptide-drug conjugates that demonstrate improved efficacy compared with peptides and small molecules independently.
