BPC-157

Definition
BPC 157, known as the Body Protection Compound, is a synthetic pentadecapeptide derived from a fragment of a protein found in human gastric juice. [1] [2] It has not been approved for use in standard medicine by the FDA or other global regulatory authorities, owing to the absence of sufficient and comprehensive clinical studies confirming its health benefits in humans. [2] Until well-designed clinical trials are conducted, BPC-157 should be considered investigational and its use approached with caution. [1]
How it works
In preclinical models BPC-157 activates several overlapping pathways, notably VEGFR2 and nitric oxide synthesis via the Akt-eNOS axis, promoting angiogenesis, fibroblast activity, and neuromuscular stabilization. [1] Systematic review of preclinical studies suggests BPC-157 enhances growth hormone receptor expression and several pathways involved in cell growth and angiogenesis while reducing inflammatory cytokines. [3] BPC-157 is metabolized in the liver, with a plasma half-life of less than 30 minutes, and is cleared by the kidneys. [3]
Evidence & status
In a systematic review of the orthopaedic literature, 36 studies were included of which 35 were preclinical and only 1 was clinical, and in preclinical models BPC-157 improved functional, structural, and biomechanical outcomes in muscle, tendon, ligament, and bony injuries. [3] In a retrospective human study of intra-articular BPC-157 injection for unspecified chronic knee pain, 7 of 12 patients reported relief for more than 6 months, but no clinical safety data were found. [3] Human data are extremely limited, with only a few pilot studies examining BPC-157 in humans, including its use for intra-articular knee pain, interstitial cystitis, and intravenous safety and pharmacokinetics. [1] Its pharmaceutical development remains rudimentary, with no approved formulation, no validated dosing regimen, and no completed Phase II clinical trial. [4]
Considerations
BPC-157 was temporarily banned by the World Anti-Doping Agency (WADA) in 2022. [2] Although animal safety studies showed no adverse effects across several organ systems, there is no clinical safety data in humans, and adverse effects are possible due to unregulated manufacturing, contamination, or unknown clinical safety. [3] The primary barrier to clinical translation is not the absence of biological activity but the absence of fundamental pharmaceutical science, including characterized formulations and validated pharmacokinetics. [4]
