Regenerative Medicine
Definition
Regenerative technologies strive to boost innate repair processes and restitute normative function, and their deployment into practice is poised to drive radical innovation in patient management to address the needs of an aging population challenged by escalating chronic diseases. [1] Tissue engineering and regenerative medicine encompasses therapeutic strategies for clinical application that include the use of biomaterials, stem cells, and growth factors to improve or regenerate different tissues. [2] A regenerative medicine model of care entails scalable production and standardized application of clinical-grade biotherapies for cell-based, acellular, and biomaterial applications, supported by comprehensive supply-chain capabilities that integrate sourcing and manufacturing with care delivery. How it works
In the context of stem cell transplantation, it is widely recognized that the main mechanism for the regenerative potential of mesenchymal stromal cells is not their differentiation but rather their secretome, that is, the paracrine factors these cells secrete, with in vivo data revealing transient and low engraftment rates. [3] Exosomes, small extracellular vesicles rich in bioactive molecules such as proteins, lipids, and nucleic acids, mediate intercellular communication and can promote skin cell proliferation and collagen synthesis while reducing oxidative stress. [4] Platelet-rich plasma therapy is based on the principle that platelet growth factors support the three phases of the wound healing and repair cascade, namely inflammation, proliferation, and remodeling. [5] Decellularized organs and tissues yield extracellular-matrix biological scaffolds used across a diversity of regenerative medicine purposes, though almost every decellularization approach causes some disruption of the tissue's three-dimensional organization. Role in longevity
Mesenchymal stem/stromal cells and their exosomes have great potential for tissue repair and regenerative medicine, and can improve the symptoms and prognosis of aging-related diseases and potentially slow the aging process through multiple pathways. [7] Advances in regenerative medicine, gene editing, and organ cross-talk modulation are contributing to the development of personalized, multi-targeted anti-aging therapies that aim to delay or reverse aging by targeting specific hallmarks of aging. [8] Recent advances in regenerative medicine have shifted the focus of cosmetic dermatology from temporary aesthetic improvements to long-term interventions aimed at preserving skin vitality and longevity. Evidence & applications
Regenerative medicine approaches spanning stem cell therapies, three-dimensional organoids, and three-dimensional bioprinting are poised to create patient-specific tissues that reduce rejection and enhance graft longevity in transplantation medicine. [10] In a multispecialty consensus guideline, use of injectable biologics in well-selected individuals with certain chronic pain indications may provide superior analgesia, functionality, and quality of life compared with conventional medical management or placebo, though many aspects remain controversial. [11] Global Vascular Guidelines recommend that regenerative medicine approaches such as cell and gene therapies for chronic limb-threatening ischemia should be restricted to rigorously conducted randomized clinical trials. [12] A critical review of the clinical applications of exosomes concluded that, while significant progress has been made in understanding exosome biology, addressing challenges in isolation, purification, and regulatory standardization is crucial for their successful application in clinical practice.
