Methylene Blue

Definition
Methylene blue is a long-established drug with complex pharmacology and multiple clinical indications, whose diverse mechanisms of action underlie a large variety of clinical effects. [1] As the first fully synthetic medicine, it has classically been used for surgical staining, malaria, and methemoglobinemia. [2] [3] It is a redox-active phenothiazine dye with reversible redox behavior compatible with biological redox systems. [4]
How it works
Methylene blue can reroute electrons in the mitochondrial electron transfer chain directly from NADH to cytochrome c, increasing complex IV activity and promoting mitochondrial activity while mitigating oxidative stress. [5] As a mild-redox agent it readily cycles between oxidized and reduced forms using specific mitochondrial and cytosolic redox centers, a proposed mechanism of electron-carrier bypass that improves mitochondrial function and prevents reactive oxygen species production. [6] In vasodilatory shock, methylene blue acts as an inhibitor of soluble guanylate cyclase, an enzyme in the nitric oxide signaling pathway of vascular smooth muscle, thereby reducing nitric-oxide-driven vasodilation. [7]
Evidence & status
Its established evidence-based use is as the main antidote for elevated methemoglobinemia, given intravenously at a dose of 1 to 2 mg/kg. [8] In a meta-analysis of six randomized trials in 302 septic-shock patients, methylene blue may reduce short-term mortality, duration of vasopressors, and hospital length of stay based on low-certainty evidence with no increase in adverse events. [9] For neuropsychiatric use, methylene blue has documented antidepressant, anxiolytic, and neuroprotective properties in both animal and human studies, with promising results in bipolar disorder, though caution is needed given its inhibition of monoamine oxidase A. [1] Randomized clinical trials in Alzheimer's disease report that methylene blue has shown enhancements in cognitive function and reductions in beta-amyloid plaque accumulation, but much of the neuroprotective and anti-aging evidence remains in in vitro and animal models. [10] [5]
Why it matters
Because mitochondrial dysfunction is a seemingly unifying pathological phenomenon across a wide range of neurodegenerative disorders, methylene blue is positioned as a promising candidate for neuroprotection. [5] Its anti-oxidative properties and ability to bypass mitochondrial complex I/III activity have renewed interest in it as a potential anti-aging drug for age-related conditions including neurodegeneration and skin aging. [2] Safety caution is required because methylene blue should not be used in glucose-6-phosphate dehydrogenase deficiency until a diagnostic test has been performed, given the risk of hemolysis. [11] [12]
