Definition
Heart rate variability consists of changes in the time intervals between consecutive heartbeats, called interbeat intervals, and a healthy heart is not a metronome but shows complex and constantly changing oscillations. [1] HRV measures the variation in the time interval between successive heartbeats, reflecting the influence of the autonomic nervous system on heart rate changes. [2] It is one of the indicators of many pathological conditions related to cardiovascular health and provides reliable information about the interaction of the sympathetic and parasympathetic nervous systems. [3]
How it works
The analysis of the variation of heart rate is a well-known non-invasive technique to identify the functioning of the autonomic nervous system, whose sympathetic and parasympathetic divisions act on the sinoatrial node. [3] HRV is able to index cardiac vagal tone, which represents the contribution of the parasympathetic nervous system to cardiac regulation. [4] During chronic stress the sympathetic nervous system is hyperactivated, and HRV is impacted by stress, supporting its use for the objective assessment of psychological health and stress. [5]

How it's measured
HRV can easily be determined from ECG recordings, producing time series of RR-intervals that are usually analysed in time and frequency domains. [6] Metrics include time-domain indices, frequency-domain values that calculate signal energy within component bands, and non-linear measurements that quantify the unpredictability and complexity of a series of interbeat intervals. [1] The ease of HRV collection should not obscure the difficulty of interpretation of HRV findings, which can be easily misconstrued but can be controlled through correct methodological processes. [4]
What levels mean
Lower HRV parameter values were significant predictors of higher mortality across different ages, sex, continents, populations and recording lengths in a meta-analysis of healthy and patient populations. [7] In that meta-analysis, studies comparing the lowest quartile of 5-minute root mean square of successive differences (RMSSD) versus the other quartiles yielded a combined hazard ratio of 1.56 (95% CI 1.32-1.85). [7] Despite many efforts there are no generally accepted HRV reference values yet, though some large studies provide values that may be used for orientation purposes.
